Submit Questionnaire

This questionnaire will ask some questions about your existing ownership transition plans.

Submit Financial Statements

This form will ask for profit & loss statements (also known as income statements) for the past five years and a balance sheet from your most recent year end.

If the agency has existed for fewer than five years, you only need to submit financials for those years in which the agency has been operating.

Submit a Book of Business Report

Please provide a book of business report (also known as a policy master, policy detail, or policies in force report). The form linked above will provide more specific instructions on which fields to include in the report.

Certain documents can provide important info with regards to your agency's perpetuation. Please review the items requested on this form and submit any documents that are applicable to your agency.

Submit Employee Information

In order to understand your staffing structure and compensation, please provide a staff roster by downloading our template and providing the information requested. If you keep your own roster that contains employee names and annual pay info, you can upload that in lieu of manually filling out our form.
