Valuation Update Forms

We are thrilled to have the opportunity to provide you with an update to your fair market valuation! Help us get started by providing data using the forms below. Please feel free to resubmit any of these forms as many times as you'd like if you cannot get everything we ask for at once.

Submit Financial Statements

In order to complete your valuation update, we will need the yearly profit and loss statements for each year that has passed since your last valuation, as well as an updated balance sheet for the agency.

Complete the Employee Info Form

This form will provide us with information about any staffing or compensation changes since your last valuation.

Upload Carrier Reports

In order to confirm the amounts in your book of business report/form and to estimate your updated retention rate & loss ratio, we need year-end carrier reports for last year from your top 5-10 carriers. If you cannot get year-end reports, more recent reports are okay.

Submit Book of Business Report

Since your book of business has probably changed since your last valuation, we will need an updated book of business report from your agency management system.

Complete the Book of Business Info Form

If you do not have an agency management system or the information in your agency management system is inaccurate, please submit the book of business update form instead. If you submit this form, you do not need to upload a book of business report from your AMS.
