All forms can be resubmitted as many times as you'd like. You will receive an email confirmation after each submission that lets you know what items are still outstanding.

Agency Owner Questionnaire

This form asks general questions about your agency's organization and operations. Not sure if you can complete all of it right now? No worries, forms can be submitted as many times as needed.

Submit a Book of Business Report

Generate a book of business report of active policies using your Agency Management System and submit it using this form link.

This report should include: Policy #/ID, Policy Type, Client name/ID, Effective Date, Expiration Date, Producer, Account Manager, Written Premium, Written Commission, Billing Company (Carrier/MGA/Broker), Issuing Company (Carrier)

Submit Carrier Reports

Please submit the most recent year-end carrier reports for your top five (or greater than 50% of  your premium volume) carriers using this form.

Submit Financial Statements

Please use this link to submit your three to five years of agency profit and loss statements.
