All forms can be resubmitted as many times as you'd like. You will receive an email confirmation after each submission that lets you know what items are still outstanding.

Agency Owner Questionnaire

This form asks general questions about your agency's organization and operations. Not sure if you can complete all of it right now? No worries, forms can be submitted as many times as you'd like!

Submit a Book of Business Report

Generate a book of business report using your Agency Management System, and then submit it using the form linked above

OR

Complete the Book of Business Information Form

This form will ask you to manually input some summary information about your book of business, including your top accounts and your top carriers. You do not need to complete this form if you have already submitted a book of business report from your AMS. It is available in both PDF and Word format.

Complete the Employee Information Form

This form asks questions regarding employees currently or recently employed at your Agency. It also asks some general questions about your commission and compensation structure. It is available in both PDF and Word format.

Submit Financial Statements

This form will ask for five years of income statements (or as many as you have) and a year end balance sheet from your most recently completed fiscal year

Submit Supporting Documentation

This form will ask for carrier reports, cluster/alliance/aggregator/network agreements, producer/employee agreements, and five years of tax returns (or however many you have, if agency has existed for less than five years)
